Raytheon 390 Premier I, North Las Vegas, May 27, 2004–A routine weather report advised that the wind was from 160 degrees at 15 knots gusting to 20 four minutes before Premier N5010X landed on Runway 7 at North Las Vegas Airport (VGT) and overran the runway.
After buying Midcoast Aviation in March, Jet Aviation continued to use the Midcoast name for the FBO but added the second barrel, “A Jet Aviation Company.” On September 28, however, the FBO was renamed Jet Aviation St. Louis at an inauguration ceremony celebrating the new name and the facility renovations. Cessna 441, Vestavia Hills, Ala., Dec. 10, 2003–After taking off from Birmingham, Warrington Development’s Cessna 441 descended in a spin and crashed in a creek ravine. The ATP pilot and his passenger were killed, and the airplane was destroyed.

General aviation pilots who report weather encounters to NASA’s Aviation Safety Reporting System (ASRS) between April and September might receive a follow-up request to participate in a voluntary written survey. ASRS officials will use the survey to generate a special study to gain data for developing preventive measures.
