BEECH King Air 200, Strasburg, Colo., Jan. 27, 2001–The NTSB determined the probable cause to be the pilot’s spatial disorientation resulting from his failure to maintain positive manual control of the airplane with the available flight instrumentation. Contributing to the cause of the accident was the loss of aircraft electrical power during IMC.

Piper PA-31T1, Graham, Texas, Nov 12, 2001–The NTSB determined the probable cause was the pilot’s failure to discontinue the approach after encountering IMC, resulting in controlled flight into terrain. Contributing factors were the dark night condition, low ceilings and reduced visibility.

Swearingen SA-227-AT, Beaver Island, Mich., February 8, 2001–Two people were killed, two seriously injured and two sustained minor injuries when the SA-227 (N318DH) crashed into trees while circling to land during a night, nonprecision instrument approach at Beaver Island Airport (SJX). The airplane was on a Part 135 charter flight operated by Air Taxi & Commuter Northern Illinois Flight Center.

Noting that about two-thirds of all general aviation accidents that occur in IMC are fatal, the NTSB recently completed a study to better understand the risk factors associated with such accidents.

The Board used “case control methodology,” which compared a group of accident flights to a matching group of nonaccident flights to identify patterns of variables that distinguished the two groups from each other.
