The NTSB is investigating the April 28 incident at Houston Hobby in which a Bell 407 operated by Helicopters, Inc. and a Southwest Airlines 737 nearly collided. Both aircraft were departing in day VFR.

In its annual review of most wanted transportation safety improvements, the NTSB updated its list of items requiring rapid attention and added a new urgent concern to the aviation category as a result of the investigation into last year's Colgan Air crash.
Bell 222, Aurora, Ill., Oct. 15, 2008–The crash of the medevac helicopter into a radio tower was caused by the pilot’s failure to maintain clearance from the 734-foot-tall lighted structure due to inadequate preflight planning and a flight route too low to clear the tower, according to the NTSB. The helicopter was destroyed in the collision and ensuing fire, killing the pilot, flight paramedic, flight nurse and infant patient.
An air traffic tower controller and his supervisor at New York JFK Airport were placed on administrative leave last month following an incident in February when two of the controller’s children were permitted to issue instructions to pilots on an ATC frequency during atwo-day span.
The NTSB ruled that pilot distraction was the cause of last fall’s overflight of its destination airport by a Northwest Airlines flight and the crew’s failure to maintain radio communications. The Safety Board also noted some deficiencies in air traffic control procedures, which it believed contributed to the duration of the event.
On August 8, a tour helicopter climbing from a Hudson River heliport in Manhattan collided with a Piper PA-32 Lance piston single. In the ensuing crash, both aircraft were destroyed, and all nine people–three on the Piper and six on the Eurocopter AS 350BA–were killed.
