The FAA on Wednesday issued an airworthiness directive (AD) requiring inspection of Boeing 787-8 engine fuel feed manifold couplings after receiving reports of fuel leaks on two in-service aircraft.
McDonnell Douglas MD-369D, Volcano, Hawaii, June 15, 2003 – The commercial pilot and three passengers were killed when the MD-369D lost power and crashed onto a rugged hardened lava flow in the Volcanoes National Park. The NTSB determined that the loss of power, and the accident, was caused by the fatigue fracture and separation of the compressor coupling adapter.

On Dec. 1, 2000, a nonfatal helicopter accident in Rockport, Texas, became the first in a series of events that culminated recently in settlements of lawsuits involving the helicopter’s Rolls-Royce 250 engine. The accident occurred when the engine’s compressor adapter coupling failed, decoupling the compressor from the turbine.
Bell 206B JetRanger, Cleveland, Ohio, June 25, 2005–A failure of the main drive shaft for undetermined reasons, the NTSB concluded, caused the total loss of engine power in the Bell 206B JetRanger. A factor in the accident was the pilot’s improper flare.
A lawyer who filed a lawsuit on behalf of family members killed in a June 15, 2003, helicopter crash began taking depositions from defendant Rolls-Royce in March. Attorney Richard Genter of Jenkintown, Pa., filed the lawsuit on May 12 last year in the U.S. District Court of Hawaii. Genter is representing the families of the three passengers killed during a sightseeing flight in an MD 500D over Volcanoes National Park near Hilo, Hawaii.
