Eurocopter AS 350BA, Princeville, Hawaii, March 8, 2007–The NTSB has blamed the fatal crash on the failure of maintenance personnel to properly tighten the flight control servo lower attachment clevis and reinstall a functioning lock washer, which resulted in a flight control disconnect and complete loss of control.

Aerospatiale AS 350-BA, Princeville, Hawaii, March 8, 2007–The NTSB determined the probable cause of the AS 350 crash was the failure of maintenance personnel to properly tighten (torque) the flight control servo lower attachment clevis and reinstall a functioning lock washer, which resulted in a flight control disconnect and complete loss of helicopter control.

En route from Wycombe Air Park, northwest of London, to Shoreham on the south coast of England, a Hughes 269C broke into two sections, with the cockpit appearing to separate from the combined engine and tailboom assembly, according to witnesses. The bodies of two of the occupants were found in the wreckage and a third was found about 50 yards away.
