Eurocopter AS 355F1, Andover, England, Dec. 2, 2003–G-XCEL was on a test flight after it had been fitted with overhauled rotor drive gearboxes. Witnesses heard a “loud screeching mechanical noise,” then saw the tailboom appear to fold forward alongside the cabin and saw the helicopter fall to the ground. The pilot was killed, as were the two maintenance engineers on board.

Reims Cessna F406, Inverness, Scotland, Oct. 22, 2004–The pilot of G-TWIG was positioning empty to Inverness. All was normal until he started his letdown, when radar recordings showed an unsteady rate of descent, as well as a track deviation, then radar and voice contact was lost.
Raytheon Beech King Air 200, Dulles, Va., Oct. 26, 2004–The NTSB said the accident was caused by the pilot’s improper in-flight decision to continue an instrument approach and landing. Below-minimums weather conditions were a factor.
Cessna CitationJet 525, Old Bridge, N.J., July 17, 2005–The NTSB blamed the crash of the Cessna CitationJet on the pilot’s improper preflight planning, which resulted in an overweight landing.
Aero Commander 690, Temecula, Calif., Oct. 13, 2001–The NTSB determined that the probable cause of the accident was a misrigging of the engine controls that resulted in an asymmetric reverse thrust condition.
Swearingen SA226TC, Casper, Wyo., Feb. 20, 2006–In night VMC, the commercial pilot of Spendair Flight 1713 lost directional control taking off from Natrona County International Airport and the left propeller hit a runway marker light. The pilot said he had set power at 60 percent rpm and released the brakes. The airplane went straight down the runway for about 800 feet, then veered to the right.
Cessna 208B Caravan, Vilanculos, Mozambique, June 30, 2006–The Caravan charter was approaching Vilankulo Airport at night when it crashed about 1.2 miles from the airport. At the time of the approach, the pilot said he thought he had experienced a loss of power. No significant weather was reported. The pilot was killed and the copilot and a passenger were seriously injured.
Cessna Citation 560, Cresco, Iowa, July 19, 2006–The pilot and copilot of Citation N636SE, registered to Tomco of Nashville, Tenn., were killed on landing at Ellen Church Field Airport (CJJ). The airplane was substantially damaged when it evidently overran the runway and came to rest in a corn field north of the departure end of Runway 33.
Cessna 560 Citation Ultra, Hamilton, Mont., July 10, 2006–According to the FAA report, Citation Ultra N50CV was landing at Ravalli County Airport when it went off the end of the runway and into a creek. The two crewmembers were not injured. Weather was not reported. The flight originated at Visalia, Calif.
Bell 206L-3 LongRanger, Taos, N.M., July 12, 2006–An NBC Channel 4 Eyewitness News helicopter, operated by AeroWest Helicopters of Albuquerque, N.M., was assisting in an air search for a missing person. The LongRanger had taken off from Double Eagle II Airport and landed at Taos Municipal Airport to refuel. It took off with approximately 525 pounds of fuel to return to the Wheeler Peak Wilderness Area.
