Piper PA-46-350P Malibu Mirage, Atlanta, Dec. 3, 2006–The NTSB attributed the Mirage accident to the airplane design stress limits being exceeded when the pilot lost control during cruise after his attention was diverted to the right-side attitude indicator, which the pilot reported as “tilted.” The pilot was looking at the indicator when he realized the airplane was in a dive.

Dassault Falcon 900EX, Teterboro, N.J., March 3, 2006–The NTSB blamed the Falcon’s overrun at Teterboro Airport on the copilot’s failure to touch down at the proper point. Factors were the gusty wind conditions (19 knots, gusting to 26), the failure of the flight crew to use the air brakes and a partially contaminated runway.
Cessna 525B CitationJet CJ3, Stuart, Fla., March 8, 2008–The NTSB blamed the crew’s failure to reset the brake circuit breaker before taxiing for the airplane’s collision with an unoccupied parked airplane when the brakes didn’t work. The first officer had pulled the brake circuit breaker two days earlier when updating the Jeppesen database to keep the brake hydraulic motor from continuously cycling.
Embraer EMB 170, Dulles, Va., May 30, 2006–Improper servicing of the nose-gear strut and inadequate maintenance caused the failure of the United Express EMB 170 nose gear to extend on landing, said the Safety Board. A factor was the operator’s inadequate checklist.
Bombardier Learjet 36, San Diego, Calif., Dec. 1, 2006–The in-flight loss of one of the Learjet’s elevators was caused by the pilot’s failure to maintain control while maneuvering, which resulted in exceeding the aircraft’s design stress limits, according to the NTSB.
Hawker Beechcraft Hawker 800XP/Schleicher ASW27-18, Smith, Nev., Aug. 28, 2006–The NTSB blamed the midair of the NetJets-managed Hawker and the glider on the failure of the glider pilot to use his transponder and on the high closure rate of the two aircraft, which limited each pilot’s opportunity to see and avoid the other.
Hawker Beechcraft King Air 200, Leonardtown, Md., Oct. 12, 2006–On approach to Runway 29 at St.
de Havilland Canada DHC-6-100 Twin Otter, Sullivan, Mo., July 29, 2006–Witnesses reported that they saw “flame emitting from the right engine” while the turboprop was taking off from Sullivan Regional Airport on a skydiving flight in VMC.
Hawker Beechcraft Beech 99, Moline, Ill. Feb. 18, 2008–The Suburban Air Freight cargo airplane had an in-flight fire in the wiring behind the instrument panel, causing minor damage. The pilot, who was not injured, put out the blaze with a fire extinguisher.
ATR 42-300, Los Conejos, Venezuela, Feb. 21, 2008–All 46 people aboard the Santa Barbara Airlines flight died when it crashed in a national park in the Andes at an altitude of 13,500 feet. The airplane had taken off from Merida, en route to Caracas. Fog was reported at higher altitudes.
