Cessna S550 Citation S/II, Dillon, Mont., May 3, 2007–The NTSB did not determine the reason for the in-flight loss of control of the Citation, which crashed on descent to Dillon Airport, killing the ATP-rated pilot and a passenger. The aircraft was destroyed.
Dassault Falcon 900, Santa Barbara, Calif., June 10, 2007–The Safety Board blamed the Falcon accident on the erroneous gross weight calculation and improper trim setting by the captain, which resulted in the airplane’s failure to rotate at Vr as expected and the captain’s decision to abort the takeoff at high speed.
British Aerospace HS.125-700A, Fort Lauderdale, Fla., Nov. 1, 2006–The Hawker landed gear-up at Fort Lauderdale/Hollywood International Airport because the flight crew failed to extend the landing gear, according to the NTSB. A contributing factor was the inoperative audible landing gear warning system. The pilot explained that he was distracted by trying to locate the runway for a visual approach.
Bombardier Learjet 35A, Goodland, Kan., Oct. 17, 2007–The Jagee Ventures Learjet broke out of the clouds at 250 feet agl on approach to Runway 30 at Renner Field, slightly left of centerline. The pilot, in aligning the airplane with the centerline, experienced an uncontrolled left and right rolling motion.
Bell OH-58C, Quitman, Ark., Nov. 16, 2008–A passenger in the OH-58C was killed but the commercial pilot uninjured when the Arkansas Game and Fish Commission helicopter landed hard in a field. The night surveillance flight supported ground personnel searching for “spot-lighters” illegally hunting deer at night. The pilot, who was wearing night vision goggles, was hovering at 150 feet agl when the low rotor rpm light came on.
Hawker Beechcraft King Air A100, God’s Lake Narrows, Manitoba, Nov. 22, 2008–The Canadian-registered King Air ambulance flight was taking off from God’s Lake Narrows Airport in night VMC when it crashed into trees. The aircraft was destroyed by the resulting fire and the three flight crewmembers and two passengers were slightly injured.
Bombardier Learjet 45, Mexico City, Mexico, Nov. 4, 2008–The NTSB has reported that the Mexico City crash of the Secretairia du Gobernacion of Mexico Learjet that killed the Interior Minister and seven others on board (see AIN, December, page 47) also killed 14 people on the ground (up from earlier reports of five) and injured 40.
The aircraft was on final approach to Mexico City Airport.
Bombardier DHC-8-311 Dash 8, Philadelphia, Nov. 16, 2008–The nosegear of the Piedmont Dash 8 failed to extend on approach to Philadelphia International Airport and the crew made a go-around to try to get it down. Unable to correct the problem by performing the alternate landing gear extension procedure, the crew landed on the main gear and attempted to delay the nose from touching down until the slowest speed possible.
Construcciones Aeronauticas SA CASA 212-200, Toksook Bay, Alaska, Nov. 1, 2008–The Arctic Transportation Services CASA 212 made a forced landing on the tundra when the captain was unable to maintain altitude after the right engine experienced a problem. The airplane was on a VFR approach to Toksook Bay, when the copilot added power. The right engine failed to respond, and the aircraft yawed right.
